HOUSEHOLD INCOME SURVEY

Completion of the annual Household Income Survey, which replaced the previously-required "Free and/or Reduced-Priced Meal" application, assists the District with important funding sources, such as telecommunication discounts, general funding and test exam discounts. All information is confidential. Submit ONLINE on the Campus Parent Portal. Note, the survey cannot be completed through the Student Campus Portal. To complete on paper, use the Household Income Survey link to complete, print and submit via email to bhdist@bhuhsd.k12.ca.us or in-person to the school office. Questions can be directed to Christine Fouts in the Business Department. If you need assistance with your Campus Parent Portal account, please contact Technology.

To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
